I once watched a lab spend three weeks chasing a split peak on an HPLC-UV method before anyone thought to check the vial. The autosampler needle was bottoming out at 0.8 mm above the base, the vial was a 12 x 32 mm with a 15 mm body height and a 4.6 mm ID, and the draw volume was 250 µL.
